Incoming Student-Athletes & Parents,

The Wittenberg University Sports Medicine staff would like to take this opportunity to welcome you to Wittenberg University. Below you will find the items that every incoming student-athlete must complete BEFORE BEING CLEARED TO PARTICIPATE in any team activities.

EVERYTHING MUST BE COMPLETED AND RECIEVED BY JULY 17^th.

Incoming Student-Athlete Forms

   • Please note some forms require a signature from a qualified medical professional.
   • These forms must be mailed to the address below or uploaded to the Deposited Student Portal by JULY 15^th to allow adequate time for all forms to be reviewed and processed.

3. ImPACT Test – Baseline Concussion Testing. Click Here to Download Instructions
4. If you have ever sustained an injury/illness that required special attention (physician evaluation, surgery, x-ray/MRI, physical therapy, etc), you must provide documentation, office note(s) or a clearance note for each injury/illness.  Further instructions can be found in the First Year Student-Athlete Health Information Packet.  Failure to provide sufficient documentation of past injuries/illnesses may delay your clearance for participation.

Forms Review Process

Your Wittenberg University Sports Medicine Department Certified Athletic Trainer (ATC) and Wittenberg University Athletic Team Physicians will review your First Year Student-Athlete Health Information Packet, SportsWare Medical Profile, Baseline ImPACT test and (if Necessary) additional documentation for injuries/illnesses in as timely manner as possible. You may be contacted if additional information is needed in anticipation of medically clearing you for participation.

Obtaining your Clearance for Participation

You will receive your clearance for participation during an arranged on-campus meeting setup by your Wittenberg University Sports Medicine Department Certified Athletic Trainer (ATC). Your coach will notify of this time and date.

The Wittenberg Sports Medicine office WILL NOT provide you with a physical exam from a physician. You must complete a physical exam BEFORE coming to campus.
